I would have had to leave either my make up or my computer and I was unwilling to part with either one! Hue works at the airport in HCM City so she met me to pick up the clock.<br />

I had an 8 hour layover in Manila. On the leg from Manila to SF, I had paid extra ($50) to have an exit row so I would be able to stretch my legs during that 12 hour flight. This plan worked great on the way to HCM City but not so well on the way home. There was a couple in the seats next to me and all 3 of us FROZE during the flight. We tried blocking the cold air with magazines, carry on bags, the little pillows they give you but nothing worked. I even had a small blanket in my carry on but that did little to help. The flight attendant said that sometimes those areas are too hot and sometimes too cold. I will not be taking that chance again unless I bring Ugg boots and a down full length coat. Even my nose was freezing cold!<br />
I arrived in SF about 7:30 p.m. on Saturday so on this trip, I had no March 2nd and two March 17ths--and, not 1 green beer to show for it!<br />
<b>Professional: </b>The students have until March 24th to finish their final paper. Our UMP faculty will review them first with special focus on format congruent with UMP's style guide. Then they will email them to us for final grading. This has been an amazing experience which has enriched my worldview. On a practical level, it has given me ideas for course revisions at NWU.<br />

She had 2 gifts that she wanted me to carry home to Mindy but I could not fit them into my luggage (one was a large picture that she had cross stitched and had framed; the other, a cross stitched picture that was then made into a clock). When we left the hotel, Hue led me to her motorbike! Yes, on my last night in VN I put on a helmet and was driven all over HCM City on the back of a motorbike. You are able to see so much more from that vantage point when compared to the inside of a cab but I can also see why most people here wear face masks--the exhaust fumes were pretty bad. We went to her friend's restaurant and enjoyed excellent Pho [pronounced fa (short a sound)]. It is probably my favorite VN food. <br />

We split into our smaller groups for the presentation of the students' development of a nursing theory. The presentations were very well done. I think the students were surprised that they were able to actually develop their own theory. This is a good assignment; I plan to implement it in my own NU 515 Nursing Theories class. <br />

<strong>Personal: </strong>The students planned a Going Away Party. It was held at the hotel restaurant where we had enjoyed wonderful food 2 times before. They presented Dr. Kay and me with lovely gifts and well wishes for safe travel. The students were so kind to us--making sure we were safe and well fed the whole time we were here! They are wonderful people and excellent students. Our 2 UMP faculty were very helpful. I trust that they learned some valuable teaching strategies from us. Dr. Kay was a most helpful mentor. Even though I had read her book, learning the application in person from the source is so much better. <br />

<strong>Professional: </strong>Students did their Nightingale presentations during our morning sessions. They did very well. The application to Vietnamese healthcare was particularly interesting as Nightingale is very appropriate for his population at this point in time. The students told of patients having to share beds--literally 2-sometimes 3 patients in one bed. In the afternoon, Kay and I met with the Chief Nurse Executive of the near-by hospital. She brought up Nightingale's philosophy and pointed to a picture on her wall. She went on to tell us that her hospital has approximately 2000 beds with an average daily census of around 4000 patients. We can all do the math. She very sadly said that she has tried to figure out what to do about this issue but has yet to come up with a solution. She spoke of patients with incisions laying in bed next to another patient! She also said that the nurse-patient ratio on the wards at night is about 40-50:1 and in the ICU/CCU, typically 7:1. I don't remember if she told us or if one of our students told us, but they do not have heart monitors in the CCU. We did not get to tour the whole hospital but they did take us to see the Emergency Department. I am having difficulty finding the words to describe the scene; "controlled chaos" come to mind. There are no cubicles--just rows of carts with a narrow aisle between the rows. I did see a couple of carts with 2 obviously unrelated patients sharing the space. The Head Nurse of the ED said that they typically staff with 20 RNs per shift and I think she said about 300-350 patients per 24 hours but I am not sure I remembered that figure correctly. Seems like so many but I think that is what she said. It makes sense in that there were at least 50 patients there during our visit. Unfortunately, I was not able to take pictures. They didn't say, "No" but I sensed that they were not comfortable with the idea. One of our students had some pictures that would be emailed to us. <br />
<br />
Thursday was spent working with the students on their theory development presentation. They worked very hard and I think will do well with this work. Thursday evening, the Assistant to the Dean, and a graduate of the UMP MSN program took us out to dinner. Kay had her as a student when she taught here the last time. They are very anxious to get a PhD program launched at UMP. <br />

All of class time was used yesterday for 18 presentations of a concept analysis. Kay and I each graded each presentation. Then last evening we compared our scores. We think our inter-rater reliability was in the high 90s although we did not run any type of official stats to confirm. For all but about 3 of the scores, we were within 1 point out of 100. The original plan was to have half the class present the theory they developed on Thursday and the other half on Friday. However, that would mean that 9 of the groups would only have one afternoon to work on this with Dr. Kay and me to assist/advise them. This did not seem at all fair to the students. So, we are going to use Thursday to work on this assignment and then on Friday, Kay will have her 9 groups present; my 9 groups will present in a different classroom. She will grade her groups and I will grade my groups. We are both comfortable with this arrangement in part because it seems that our grading is very similar.<br />

We showed the Florence Nightingale movie in class today. Had a bit of a problem with the sound. Kay had to hold one of the microphones up to the speaker on the laptop for the students to be able to hear! She figured out how to use the subtitles on the movie which helped alot! I will do the same with my own classes as there are times when Flo speaks too softly. One of the students felt that the movie was so helpful in making the theory 'come alive'--my interpretation, not his words. He said that he would like to be able to show it to his own nursing students.<br />
<b>Personal: </b>Several students took us back to a restaurant that we loved for lunch today. There is a wonderful buffet and the service is excellent. I accidentally ate some type of liver. Those of you that know me well know what a mistake that was!! After we returned to the hotel, Kay and I took a short walk around the neighborhood and through a local market. You could certainly buy fresh food there for cooking your supper. It is slightly dangerous because motorbikes are constantly driving through the middle of the market. You can just drive up on your bike and choose your food. We also saw a man getting a shave at a sidewalk barber chair. <br />

A group of our students had arranged a van for the trip. We stopped along the way at what I would say was analogous to a truck stop to eat breakfast. They brought wonderful snacks for us to share. My favorite was the grapefruit. I have never loved grapefruit but this was delicious--sweet and a little dry. </div>The wind was so strong that my sun hat was nothing but a nuisance. One good sized wave managed to take me clear under but I didn't mind a bit. There were also 2 swimming pools there--one a salt water pool which really didn't make much sense to me. Kay and I swam in both.<br />

She came to the hotel to pick up the treats that Mindy sent her--Peanut M&M's, Dove deod., and some "healing oil" that is available in the Vietnamese store in Lincoln. Hue and her boyfriend took me out to dinner for a traditional VN meal. Then I made my way back to the hotel in a cab. <b>Personal: </b>Attended mass at the cathedral that we saw last Sunday on our tour. Church was nice--hotter than I imagine hell to be but at least they had fans at the end of each row. After a rest in the shade, we made our way to a market. Kay and I wanted to take a cab but her husband, Gayle, assured us that it was only a couple of blocks and he had met a splendid chap from England that would lead us there. Well, about 10-12 blocks later we arrived with our make up melted off our faces and hot, tired feet. Cabs are so cheap here, there is no good reason to walk in heat of 100 degrees with 85% humidity! Kay and I assured Gayle that we would be eating in an air conditioned, sit-down restaurant. I think he was tired of hearing two strong-willed women complain so he readily agreed.<br />

The market is...well, overwhelming! There is so much "stuff", narrow rows, lots of people, many different smells from the food as well as the fabrics, and who knows what else. My students told me that I had to bargain when I went to market so bargain I did. The trick is to ask for the price in Vietnamese Dong (VND). Some will tell you the price in dollars but this is always much more than you would pay if you negotiate in VND. I got a little confused once but Kay caught me before I handed over too many dong. She has traveled quite a bit as has her husband. They have both been a great help and wonderful companions. I bought most of the gifts that I needed at the market or along the street. No toy shark for Micah but I can probably find that when I get back to San Francisco. I had a traditional VN soup--Pho--for lunch. It was delicious. One of the pictures is in the market and another is a jewelry store along the street near the market where I bought a ring. Husband Dan needs to know that it was a really good deal! ;>) <br />

<b>Professional: </b>Spent some time late in the afternoon reviewing work from two of my MSN Major Project advisees. I am trying to be as responsive as possible to keep them moving along while I am out of the country. I am so thankful for email!! Tomorrow, all 18 pairs of our VN MSN students will present their concept analysis. Then they will have a group presentation on Wednesday (I think...) on Florence Nightingale. The same pairs will use their concept to develop a simple nursing theory. I am anxious to see how this unfolds. I have always been opposed to this as an assignment for our NWU MSN students because truly developing--and testing--a theory can take years. I do see that putting together 3 concepts can be a theory and a good starting place for our MSN students who plan to go on to pursue a doctorate. I foresee some revisions to my MSN theory course.<br />

We are so thankful for our translator and our two UMP faculty. This material is challenging when it is in your first language; I can only imagine how difficult it is when it is NOT your first language. We have spent time with each of our small groups assisting with their concept analysis. Each pair + 1 group of 3 will present on Monday for a total of 18 presentations. I am thankful for the weekend to let my brain recover! <br />
<strong>Personal & Cultural: </strong>Thursday was the International Day of the Woman which is celebrated quite extensively here. Several of our male students presented us with flowers. <br />

The students were very kind with their applause. Many of the students sang traditional Vietnamese songs. <br />
After class, one of the students went with me to a nail salon to interpret so I could get my purple-dyed fingernails fixed. Somehow I ended up with a leg and foot massage that took about an hour! It was very nice but not really what I had in mind. They did get my nails fixed. <br />
<strong>Culture</strong>: I asked the students about women smoking in VN and they said it is ok. I don't know if that is true or if they only said that to make me feel better. I have seen men smoking everywhere but not women. When I have a break at the school, I go outside the gate. At first the students were concerned for my safety and would stand by watching. I told them that I would stay close to the gate with the guard in sight. The guard was very stoic--almost looked disapproving. I would always smile and nod at him but got very little response. By Wednesday, we had our UMP picture ID as a guest lecturers, so the guard was able to see that I had official business at the school. On Thursday, I finally got the slightest smile from him when I greeted him. Today, he actually came outside the gate and stood near me in what I interpreted as a protective gesture! <br />
Tomorrow some students are taking us to the seashore. I am so excited! Apparently it is a bit of a drive but we will be able to see another part of Vietnam that is very different from HCM City. In the afternoon, we divided the students into 2 groups. My group went to an upstairs classroom. The air conditioner was turned on but was apparently on fan not AC. We all had sweat pouring down our faces. Even my VN students who are accustomed to this heat were not tolerating it well at all. Our brains finally ceased functioning and we had to return to the downstairs, moderately cool classroom. Today, we turned the AC on upstairs a full 2 hours before we planned to use the classroom. This worked much better. It is important to remember that the environment impacts the ability to learn!! <br />

We covered concept analysis today and then did some exercises to help the students understand the process. I have been reading through my students' concepts. Some are a little too broad for the purposes of this course and this assignment. These students took the Master's level research class just before this course. We are having some trouble helping them find a concept related to their research ideas that is amenable to a concept analysis in a few days rather than a few years! Dr. Kay asked them to bring their research hypotheses and research questions to class tomorrow. The students will work in pairs for this concept analysis assignment and with the same person for their theory development assignment. We will meet with each pair of students tomorrow with high hopes that we can get them on the right path! :>)<br />
<strong>Personal: </strong>Yesterday a group of students took us + the two UMP faculty members to lunch at a restaurant in a hotel. They said it was very famous in HCM City. The food was wonderful. However, I could not make myself try the black snails in spite of encouragement from my students. <br />

There was a lovely ceremony with wine toasts in beautiful small crystal glasses. Some attendees has 7 Up or Pepsi in cups. We are not sure why some got wine and some not. We were presented with a gift from the Dean--a beautiful wooden figure of a Vietnamese woman. For dessert there was a very moist cake + grapes and Vietnamese pears. These are very red--they look almost like apples. They are very good--crisp and juicy. After I took a bite of mine, a look of horror came over the face of our host. She said, "Oh, no, no!" and took the fruit from my hand. When I looked where I had taken a bite, there was a cluster of small black gnat-like bugs some of which I had just swallowed. I fervently hope they are not a bug that is the host to some type of parasite!! <br />

After I returned from our afternoon classes, I washed some clothes out in the sink. Something literally dyed my fingernails a purple color. I saw a nail shop down the street so I plan to see what they can do to fix this problem. I still have no phone and Verizon does not seem to be doing much if anything to fix the problem. <br />
Kay & I had dinner at a roof top terrace in a hotel across the street. The terrace was lovely with views of the city. I am having trouble uploading pictures tonight so will add more pictures tomorrow.<br />

We went over the course guide for the next 2 weeks and discussed our plans for small group work in the afternoons. For the major paper (development of a nursing theory based upon the students' concept analysis), we decided that the UMP faculty would review the student papers first for formatting in the UMP style (similar to MLA) and their review of content. Then they will forward the papers to us for final grading. Our focus will be on the content rather than format. <br />
Our first day of class started with a broken toilet spewing clean water all over the faculty restroom! It was from a broken sprayer (similar idea to a bidet...). A quick thinking student tied a glove around the sprayer to stop the shower until maintenance could be summoned. Then we discovered that Dr. Kay's email with the PPs for the day's lectures had not reached the UMP faculty to be printed for the students. Ms. Thoa and I took over printing duties while Dr. Kay started lecture. Our translator is a nurse who is in a Master's program in Australia. I am not sure of the details--whether it is online or if she goes there for class and returns to Vietnam between. At any rate, we are so happy to have her in the classroom. To lecture, we go over 1 slide at a time and then she translates for the students. I did a short lecture on plagarism from Dr. Kay's slides. This generated lots of questions from the students. Kay gave permission for me to use it with my own students. <br />
We were asked to join the Dean of the Faculty of Nursing-Medical Technology for tea this afternoon. Tran Thien Trung, M.D., Ph.D. has been at the UMP for about 2 1/2 years. He welcomed us to the university and thanked us for working with their MSN students. He also discussed the need for a PhD in Nursing program here. He said, "Doctors cannot teach the nurses for the PhD!" I was happy to agree with that comment! The assistant dean was in Kay's class in 2009. I did not get her card so I do not have her name. Dr. Tran's assistant is in the background. <br />

The Saigon area is typically where the tourists spend time; it is in District 1. Our hotel is in District 5. Many of the shops in this area carry high end brands that we see in our stores. However, there are open air markets, inside stall-type markets, and street vendors all over, some with open fires cooking the food that they sell. Met Dr. Kay Avant, my co-faculty, last evening. I was in my lounge dress and half-asleep. I am hoping she is able to get that first impression out of her mind! I was able to sleep until 1 a.m. but then was wide awake. Decided that it would be wise to utilize the sleeping pill Rx that my Dr. gave me. I took 1/2 and was able to sleep from 0200-0600 when I swear I heard a rooster crowing outside! It's "voice" was not as robust as Dan's rooster so I think it must be a smaller variety living on someone's rooftop. <br />
My room here at the Bat Dat Hotel is quite comfortable although the electricity to the room turns off when you take your key out of the slot by the door (see picture). When you lift the key out, the electricity shuts off. Guess we could learn something about conservation from the Vietnamese people. That means when you return, it is pretty warm in your room. The hot water heater only turns on when you hit the correct switch but it doesn't take long to heat. The hotel includes a buffet breakfast free with the room fee. I chose to be safe this morning and had 2 eggs with toast. The eggs actually looked as good as Dan's--brown eggs with bright yellow yolks. I will be a little more adventurous for lunch today. <br />
<strong>Professional: </strong>As I said previously, Kay and I will meet with the UMP nursing faculty assigned to our course at 2 this afternoon. I have my lecture pieces all prepared. Kay will be the lead faculty since she has taught this course before. That makes me feel less nervous. She was happy to hear that we will have a translator in the classroom since they did not and it was very difficult. Of the 5 students total who met me at the airport yesterday, 2 spoke English well, 1 fairly well, and 2 not so much. I would guess that might be indicative of how the overall class will be. The students were very kind and respectful but fun. When I told them that I really hoped that we would be able to go to a beach one said, "You wear bikini, Dr. Linda?!" When I said, "No. This body has not been in a bikini in a lot of years!" they all laughed and tried to convince me that I should. <br />

The flight was originally scheduled to leave about 9:15 p.m. but was changed to 11:30 p.m.; consequently there were a lot of people waiting in the boarding area. I saw many elderly, somewhat disabled people being lovingly tended to by young relatives. For example, a young woman (~ 22-23 yrs old) was helping an elderly gentleman in the bathroom. He had a walker and seemed confused but she very gently tended to his needs in a respectful and loving manner. I saw similar examples on the airplane to Manila. In the middle of the night, a tiny, "bird-like" woman was obviously distressed--making almost chirping sounds--and her young female relative was calmly walking her around the plane while murmuring gently to her. The devotion to family was apparent all around me. <br />
I am looking forward to our city tour later this afternoon. The cacophy is unbelievable--horns honking constantly and motor bikes weaving in and out. I wonder how many fatalities occur on these city streets...We have to cross the street to buy bottled water later this morning; send us your prayers!! <br />

Thank heaven for a shower and a clean bed for a nap. I was able to sleep some on the leg from San Francisco to Manila, Philippines. Professional: Have completed my lectures and Powerpoints for Concept Analysis, Orem, and am working on the Theory of Unpleasant Symptoms. We have our syllabus and course guide completed and it will be sent to UMP nursing faculty tomorrow. Need to continue to work on grading my own students' theory papers--10 done and 11 to go--so I can be ready to post their final grades by Wednesday.<br />

Originally, faculty focused on not only the content of written work but also English language usage and APA style. Now the students are using the style guide of the University of Medicine & Pharmacy (UMP) which is similar to MLA style. We decided that it would be wise to utilize our UMP nursing faculty to review the written work for correct writing style and Kay and I would focus on the content. The students will work in pairs for their theory development paper and presentation. We will divide the group (37 students) in half so each of us has 18/19 students and 1 VN faculty member to assist. Mornings will be focused on lecture. In the afternoon, we will split into our smaller groups for more 'hands on' instruction.Linda K. From the personal perspective, I plan to chronicle my activities and my feelings. From a professional perspective, I plan to record my teaching experiences.<br />
<b>Preparations & Personal Thoughts</b>--Perhaps foolishly, I added up the total travel time, including layovers, to get to Ho Chi Minh City (aka Saigon): Omaha to San Francisco with short layover in Denver=~5.5 hours; SF to Guam to Manila, Philippines=16.5; Manila to HCM City-2 hr layover + 2.5 hr. flight=4.5 for a grand total of 26.5 hours travel time. WOW. I am a little concerned about 'this old girl' traveling that long. A friend told me that her son (a physician) travels a lot and he suggests using Ambien to be able to sleep on the plane. He says that it does not have the 'hangover' effect of some other sleep aids. I planned to take Benadryl but I think it would be a good idea to check with my Dr. about this idea. He also said that you should only nap for a few hours after you arrive and then get out into the sunshine if possible to help your body clock reset. I will arrive on Saturday morning, March 3rd which happens to be my birthday. I think a good nap will be the best BD gift for this year. :>) <br />
I tried to update my cell phone for international use but found that I will have to change my cell phone plan to Verizon and buy a new phone. I did not want to have to figure out a new phone but guess I have no choice. Plan to do that tomorrow. Also still need to find a calculator for money exchange. Will probably order that online and hope it arrives in time...one of the details that I forgot about. I hope there aren't any others that I have overlooked. Will get my Hep A today or tomorrow at NWU Student Health.<br />
I talked to my credit card representative and have it arranged to be able to use it on the trip. However, there is a 3% International Charge on every transaction + the unknown of the conversion rate on the day that the charge is posted. Seems wise to avoid using the credit card unless necessary. I also talked to United Healthcare to be sure I am covered outside the US. If you need healthcare you pay out of pocket and then submit for reimbursement after back in the US.<br />
Thankfully Dan will be able to drive again in about 1 1/2 weeks--just in time for his chauffeur to leave for VN!<br />
<b>Professional--</b>I will be teaching with Dr. Kay Avant. She taught this course with the last cohort. They had 14 students; we have 37! We have a phone conference scheduled tomorrow morning with Dr. Faye Hummel & Dr. Sara Jarrett, members of the Friendship Bridge Nurses Group Steering Committee. Dr. Avant says that grading the papers is very time intensive primarily because of the English language issue. What should nurses do?" In the meantime, I have 22 theory papers to grade for my current nursing theories class.<br />
<b>Misc</b>-Did not receive a repaired camera yet but daughter Rosie found my old camera + she had bought a download connection cord for it so I will not have to buy a new camera for the trip. Class Prep: Dr. Kay Avant and I have completed our syllabus and course outline. We will have 37 MSN students. I am working on my lectures (Concept Analysis/Concept Development, Orem, and Theory of Unpleasant Symptoms). Waiting to see if the Steering Committee has any revisions for us to make. I think that I need to revise my PowerPoint lectures to make sure that my students can understand what I am trying to communicate. I guess that is always the goal but even more important to break everything down to simple terms for these students with somewhat limited English language abilities.<br />
2. Travel Prep: Our spare bedroom is a jumble of clothes, suitcases, travel accessories (thank you to my children for all of these items!), and lists. I have trouble packing when I am just going out of town for a weekend let alone for a total of 3 weeks. I have received my boosters for Hep B and Tetnus/dip/p (thank you NWU Student Health Center). Still plan to get the first dose of Hep A later this week. My family doc provided a Rx for an antibiotic just in case. Still need to get my cell phone set up for International use and find out if I can use my credit card in Vietnam.<br />
3. Made what could be my worst hair-do mistake in a decade. I thought a perm would be a good idea. It is hot, humid, and rains often in VN. It<b> is </b>a wash and wear hair-do but I hardly recognize myself when I look in the mirror. Guess I will get used to it and may end up being happy to have an easy style.<br />
4. Unfortunately, my new in August camera is broken. Sent it in for repairs yesterday. Cannot find my old camera which still worked. I may need to purchase another camera for the trip.<br />
5. I am trying to mentally prepare for the long plane ride. I do not like flying. One might wonder why I would agree to a trip that includes one leg that is 16 hours long! Fear of flying (isn't that a book?...) should not prevent me from taking advantage of this wonderful opportunity. I have a pillow, blanket, eye mask, personal fan, anti-embolism socks, and some Benadryl so I should be fine. :>) <br />

He has been unable to sleep more than 4 hours a night for the past 5-6 nights and could not sleep at all 2 nights because of muscle spasms in his legs and then his arms as well. We have been in touch with the neurologist and are decreasing the dose. It is horrible to think about how much impact this injury has had on our lives--obviously and especially on Dan's! Linda K. Another at 8; another at 9:30 + 2 ibuprofen. He was pacing and rating his pain at a 10. Called his surgeon. Gave him 2 more pain pills and 2 ibuprofen at 11:15 but the pain was still 10. Had to take him to the ER in Wahoo for first IM and then IV pain medication. One of the blood pressures they took while he was still rating his pain at a 9-10 was 204/106. They kept him there for a few hours. I came home and slept for 2 1/2 hrs. and then went and picked him up. I gave him 2 Percocet at 0730 and he is finally sleeping.Linda K.
